I hereby acknowledge this release from liability for accidental injury, an undesirable result with my hair or skin, damage to clothing, which I may incur as a result of receving services at the Vancouver Hairdressing Academy, Space Salon, or Statik The Salon Inc. I hereby assume all risks connected therewith and consent to receive hair services from the Vancouver Hairdressing Academy, Space Salon, and Statik The Salon Inc.

I accept the terms of this contract under no duress and I submit this under my own free will.
